Participates in the process of developing new pharmacy ambulatory care Specialty services. Participates in direct patient care activities, and teaches classes to patients and other health care professionals. Works with a technician III to review charts and make recommendations. Provides and documents enhanced clinical services to patients within the Specialty Clinic and Amcare in collaboration with physicians to improve patient outcomes and clinic revenue. When out of scope, communicates recommendations to Medical Providers or Designated Physician of the Day. Provides pharmaceutical care to ambulatory patients. Orders lab work for patients. Interprets Laboratory results and recommends therapy based on Clinic protocols. Works in a team environment with other pharmacists, technicians, residents and nurses. Participates in the Quality Assurance for clinic activities. Creates assessments of research and publishes results, and aids in development of research for areas related to ambulatory care, specialty pharmacy, etc. Provides patient-specific analysis and consultations (oral or written) when appropriately requested.Requirements-- Possession of a valid California license as a registered Pharmacist issued by the California State Board of Pharmacy with no Restrictions required. -- Residency or BPS is preferred. -- PGY2 and experience in specialty clinics and Amcare is preferred . -- Knowledge of pharmacy terminology, generic and brand names, abbreviations, and drugs on current hospital formulary. -- Ability to interpret and understand physicians' orders, pharmacy orders, and nursing records. -- Ability to accurately perform pharmacy calculations, and knowledge of current California Pharmacy Law. Ability to operate computer programs. -- Demonstrated problem-solving and decision-making skills. -- Excellent written and oral communication skills and interpersonal skills to establish and maintain positive professional relationships with all clients and co-workers. -- Ability to exercise discretion to ensure that patient confidentiality is maintained and that a professional, respectful clinic/department environment exists at all times.
