As a Care Coordinator, you will perform a variety of casework duties and provide social work services to patients, families, and designated caregivers. Assists with gathering information from the member for the health care team, regarding barriers to following plan of care or other health information, including ongoing screening of the member for changes in health status and coaching for improved self - management. The Care Coordinator will also implement the interventions to promote improved health and compliance with recommended plan of care, as approved by the Interdisciplinary Care Team.Primary Responsibilities:Create a positive experience and relationship with the member Keep the member out of the hospital by supporting regular visits to their primary physician Keep member actively engaged with their primary physician Support the member to ensure adherence to medication plan Proactively engage the member to manage their care Provide member education Help to keep members compliant with their care plans Partner with care team (community, providers, internal staff) Knowledge and continued learning of community cultures and values Follows specific time - frames for communication, screening and facilitating of care team meetings with the member and the caregivers, and other members of the health care team to facilitate and coordinate the members approved plan of care. Maintains Care Coordination plan including, but not limited to: Transportation providers, DME vendors Physician offices / Behavioral Health provider's appointments. Documents all information gathered for the initial assessment, development of the plan, implementation, monitoring, and evaluating outcomes.
Requirements Qualifications:Must be able to work 4 10 hour shifts, Monday - Friday, 8:30 AM - 7:00 PM or 9:30 AM - 8:00 PM Must reside in the state of VA Bachelor's Degree (or higher) in Social Work OR High School Diploma / GED with Licensed Practical Nurse (LPN) certification / license Knowledge of culture and values in the local community Experience with outbound calls to members, Physician's, Pharmacy, Providers or similar medical facility Ability to create, copy, edit, send & save utilizing Word, Excel & Outlook Must be able to navigate a PC to open applications, send emails, and conduct data entry Ability to work in multiple systems and applications to complete daily tasks Must reside in a location that can receive a UnitedHealth Group approved high speed internet connection or can leverage existing high-speed internet service Must reside in the state of VirginiaPreferred Qualifications:Experience working in Managed Care, Behavioral Health and Substance Abuse Knowledge of VA Medicaid Bilingual fluency with English & Spanish Familiarity with the resources available within the communityCareers with Optum.
