Join us and start doing your life's best work.(sm) Under general supervision of the Director of Clinical Pharmacy, the Clinical Pharmacist is responsible for performing comprehensive and targeted Medication Therapy Management (MTM) in high risk patients to identify, resolve, and prevent medication-related problems. The Clinical Pharmacist documents the care delivered and communicates essential information to the patient's primary care providers and the health care team. The Clinical Pharmacist is also responsible to follow processes to improve patient care, outcomes, and quality improvement metrics related to CMS HEDIS/Star ratings and assist in the coordination of the Medical Assistance Program (MAP) for WellMed patients. Please note: This is a full time, office based position located at our WellMed office on NW Parkway in San Antonio. 1 – 4 hour Saturday shift is required every 4 weeks. Primary Responsibilities:Utilize prescription and medical claim data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocolsProvide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitutionUtilize health plan's formulary information to guide appropriate medication recommendationsKnowledgeable of current and emerging clinical trendsActive participation in various clinical departmental meetings and forums to improve clinical programs, enhance processes, and share clinical informationContinuously look for ways to streamline and improve workflowAssist with user acceptance testing for new clinical programs and system enhancementsServes as a clinical resource and consultant in all areas of pharmacy and medication managementAssist in mentoring Pharm.D pharmacy studentsActs as a consultant to WellMed Anticoagulation ClinicSupports medical management and pharmacy quality initiatives.
Required Qualifications: Doctor of Pharmacy degreeCurrent Pharmacist license to practice in the applicable state or the ability to provide proof of licensure within 90 days from date of employment2+ years' experience as a clinical pharmacistExperience working with provider relations, working within various payor environments, and risk managementAbility to travel as necessaryPreferred Qualifications: Fluent in Spanish Language a plusBoard certification by the Board of Pharmacy Specialties (BPS)Certified Geriatric PharmacistCertification in Medication Therapy Management (MTM)Experience with Medicare guidelinesExcellent written and verbal communication skillsExcellent relationship building skillsPlanning and organizational skills to demonstrate leadership and initiativeStrong interpersonal skills.
