This position supports the team by performing activities as outlined below, and as assigned by the Clinical Services Manager of the team. Sr. RN Care Coordinator performs activities that fall within the RN scope of practice. This role works in close collaboration with all of the team members, and may support multiple providers. The position works under the direction of the provider, and activities are delegated to the RN by the provider or the team CSM. **This position will be Full-time in the field assisting the NP with assessments and triaging of members in long term care facilities.** Primary Responsibilities:Diagnosis reviews for coding accuracy Provide in - services to partnering nursing home staff for development and new home openings Assist NP's with health assessments Obtain vaccination reports from nursing homes and enter information into patient electronic medical record Monitor use of level 2 bed support surfaces to meet Medicare criteria Must travel to nursing home facilities daily throughout Essex County in New Jersey.
Required Qualifications: Active and unrestricted RN license in the state of NJ 3+ years RN experience in long-term care, acute or home health settingsPreferred Qualifications:Bachelor's degree Long term care / nursing home experience Long-term care or geriatric experience Acute care experienceAlready Fortune 6, we are totally focused on innovation and change.
