This role participates in shaping the myConnections Social Determinants of Health (SDOH) strategy in the state across all lines of UHC business. The Director establishes influential relationships across the state with external service partners, prospective external customers and internal health plan leadership from all lines of business. The Director works closely with each myConnections work stream leader and their team members to ensure SDOH operational metrics, financial targets and outcome based proof points are defined by work stream to report holistically at the state and local level. This role effectively leads a successful, collaborative team of people from across the myConnections work streams to achieve the targeted SDOH metrics and proof points. The Director communicates the myConnections value story and associated SDOH metrics and proof points to internal and external stakeholders at the state and local level.The myConnections State Director has multiple responsibilities as follows:Serves as the "account manager" to myConnections health plan customers and service partners at the state, county and local levels. Building, developing, improving, influencing and expanding relationships with internal and external stakeholders across the state.Being familiar with the myConnections value story, state strategy and work stream targets, metrics and proof points in order to effectively articulate myConnections to internal and external stakeholders across the state.Bringing forward new opportunities to expand myConnections service offerings across the state by identifying prospective new customer and/or service provider relationships, new member/consumer target populations and innovative funding mechanisms.Supports UnitedHealth Group growth by collaborating with the health plan and myConnections work stream leaders to evaluate new opportunities and build a supporting business case.Manages a matrix team across work streams to achieve SDOH metrics and proof points at the state and local level. Providing leadership to all UnitedHealth Group employees in the state supporting the delivery of the myConnections state strategy (i.e., PCCM Community Health Workers, work stream navigators, and service provider contracting team).Facilitating recurring matrix team meetings to discuss progress and issue resolution required to achieve SDOH metrics and proof points; status of service partner relationships and plans to enhance; national, state and local opportunities to communicate matrix team successes; etc. Facilitating recurring Joint Operating Committee (JOC) meetings with service partners and matrix team members to discuss SDOH metrics and proof points and opportunities to enhance processes and relationships. Driving member/consumer and service partner satisfaction to positively influence Net Promoter Score (NPS).Participating in the implementation of the myConnections offerings in the state by:Working with the health plan and myConnections work stream leaders to define the hotspots and target member/consumer populations at a state and local level.Working with health plan and myConnections work stream leaders to target state and local service partners using HealthSperien data and the Director's knowledge of the state and community partners.
Required Qualifications: Undergraduate/ Graduate Degree (Behavioral Health/Social Work/ Mental Health Counseling Degree preferred). Awareness of Trauma Informed Care Philosophy5+ years of direct people management or team lead experience Ability to lead a team within a matrix environment where team members are engaged in the success of the team as indirect reports but have a direct reporting relationship to others. Experience in developing, managing and enhancing relationships with external customers and/or community partners.Dom0nstrated experience working effectively with a wide range of constituencies in a diverse community.Demonstrated empathy and compassion for consumers with socio-economic challenges.Proven success in managing professional staff and providing guidance and support to help them achieve their goals.Excellent communication and persuasion skills and demonstrated experience in presenting organizational value and outcomes to internal executive teams and external stakeholders.
