As a Clinical Pharmacist you will provide clinical consulting services to our hospice clients, perform Medication Therapy Management (MTM) reviews, prepare drug information responses, create drug teaching tools, and participate in teaching activities. You will find a rewarding team culture that provides opportunities to learn and evolve every day. ***Role will require a rotating schedule between daytime and evening shifts as early as 6:00am and as late as 10:30pm EST*** Primary Responsibilities:Communicate with hospice nurses and physicians via telephone or in person when appropriate Review medications for drug therapy appropriateness, effectiveness of therapy, duplications of therapy, interactions, monitoring parameters, side effects, and payment coverage based on client needs Provide drug information responses to client inquiries using appropriate resources Prepare drug teaching tools to assist clients with educational needs Participate in quality improvement activities Provide drug utilization evaluations to hospice clients Assist with pharmacy student activities Additional responsibilities as assigned
Required Qualifications:Current and unrestricted pharmacist licensure (any state) Bachelor's degree in Pharmacy 1+ year of experience working as a licensed pharmacist Proficiency in MS Office Preferred Qualifications:PharmD PBM experience 1 year of clinical experience and / or completion of a pharmacy residency program Hospice, Palliative Care and / or pain management experience Additional certifications such as CGP, BCPS, CPE or similar certification Careers with Optum.
