Primary Responsibilities:Coordination of care between practice and other Optum programsReview all member charts prior to a physician appointment and create alerts / triggers to highlight Star opportunities for the ProviderPartner with the Practice's Administrative and Clinical staff while managing patient appointments and data between visitsAssist members in navigating their network of Providers by assisting with follow - up and Specialist appointments as neededAssist with Program Eligibility verificationsAppropriate Referrals to Case Management staffInteract with patients and providers via telephone; scheduling appointmentsPerform data collection, data entry and quality monitoring activities, as applicableDocument case information completely, accurately, and in a timely mannerDemonstrate sensitivity to issues and show proactive behavior in addressing customer needsProvide ongoing support and education to team membersOptimize customer satisfaction, and positively impact productivityWill not conduct any evaluation or interpretation of clinical data and will be supervised by Licensed and / or Certified staffAdhere to corporate requirements related to industry regulations / responsibilitiesMaintain confidentiality and adhere to HIPAA requirementsOther duties, as assigned
Required Qualifications:High School Diploma / GED or higher Customer service experience, telephonic and / or in personExperience in data entryProficiency with a Windows PC and ability to learn new applications easilyHealthcare industry experienceAccess to reliable transportation with the ability to travel to client sites within the designated territoryUp to 75% travel Must live in Mobile or Baldwin Counties Preferred Qualifications:HEDIS knowledge and / or experienceSTARS knowledge and / or experienceEMR (Electronic Medical Records) knowledge and / or experienceAbility to type 50+ WPMCareers with OptumInsight.
