UnitedHealthcare (TeamMD) is recruiting for an experienced Addiction Therapist to work as part of an interdisciplinary team with myConnections to provide integrated health and social services to individuals who are homeless or at risk of homelessness and suffering from substance use disorder. The greatest barriers to better health for many people enrolled in Medicaid are not clinical issues, but rather social or financial barriers, such as lacking access to safe and affordable housing or having no way to travel to a job or needed services. MyConnections integrates, organizes and distributes support services for consumers and provides people with greater access to safe housing, transportation, education and job training, which are essential to good health. The Addiction Therapist will be embedded within a transitional housing complex and will work closely with an interdisciplinary team to assess individual needs, help individuals transition safely from hospital or homeless shelter, provide post-acute care, and coordinate referrals and services with local primary care and community based organizations. The Addiction Therapist must have demonstrated compassion and mission driven attributes with at least two years of post-licensure experience, at least one year of experience working with individuals living with mental illness and/or substance addictions, and awareness of working in a trauma informed care environment. Strongly prefer experience working with homeless individuals. Work environment is primarily in home (in transitional low income housing), with some field work, including homeless shelters, CBOs, etc. The ideal candidate will have experience in full scope family medicine with a special interest and familiarity in meeting the medical needs of patients with complex behavioral, social and / or functional needs. Primary Responsibilities: In consultation with other professionals, determines the rehabilitation approach that will best meet the needs of the patient Recommends and assists the patient in setting standards of behavior and goals for evaluation of progress Provides individual and group counseling, makes appropriate referrals to community or other professional resources as necessary for effective rehabilitation Conducts therapeutic short-term, in-depth counseling and may occasionally handle crisis intervention or other emergency situations Contributes to team decisions concerning necessary controls and restrictions required for the therapeutic progress and recommends degrees of supervision required Observes for patient behaviors possibly indicating difficulty adapting or responding to treatment (e.g. missed assessment appointments, failure to attend or maintain abstinence, risk to self or others, disruptive behavior, etc.) May handle crisis-intervention or address other emergency situations without benefit of specific instructions. Such extraordinary interventions will be carefully documented and communicated to the team in a timely manner Maintains confidentiality of all patients Performs other directly related duties as assigned Must be able to integrate values of justice, respect, compassion, excellence and stewardship into provision of servicesWill work with diverse care teams in a variety of settingsWill require time management and communication skills, the ability to work independently, detail oriented, dependable and reliable
Required Qualifications: Certification in Addiction Therapy 2+ years on-the-job experience in addiction therapy Must have dependable transportation and ability to travel to and from job sites Preferred Qualifications:A bachelors' or masters' degree in an appropriate field Experience in management of chronic conditions Experience working with individuals who are homeless and / or underserved Home care or home visit experience Excellent administrative, organizational and verbal skills Careers at TeamMD:.
