Has knowledge of and applies utilization criteria for the use of appropriate resources while patient is hospitalized. Collaborates with physicians and other licensed independent practitioners to provide seamless care for patients across the continuum of care. Ability to use sound clinical judgment, critical thinking and nursing process to provide a plan for patients upon discharge from the hospital. Has knowledge of community resources for skilled nursing care, acute rehabilitation care, adult foster care, home health care, durable medical equipment, hospice and other resources for patients and their families upon return home. Minimum knowledge, skills and abilities: 1. Knowledge of discharge planning coordination and utilization review at a level normally acquired through 1-2 years clinical experience in a comparable environment. 2. Interpersonal skills necessary to communicate effectively with patients, families, interdisciplinary team members, community agencies and third party payors. 3. Critical thinking and organizational skills necessary to assess and plan for patient/family post discharge needs in a timely manner. 4. Current State of Michigan nursing license. 5. Show proficiency in basic computer skills. 6. Ability to provide care for patients in the following age groups: Infant - Birth to 1 year; Child – 2 to 12 years; Adolescent – 13 to 18 years; Adult – 19 to 65 years; Geriatric – over 65 years.
