Collaborates with physicians, advanced practice providers, and ancillary health care team members to coordinate system and community resources to maximize advocacy for patient and family care in achieving the most effective outcomes. Operates from an in-depth understanding of the importance of total health management and supports the patient and family/caregiver choices.
Collaborates with providers and clinicians in identifying appropriate patients for the Health Coach Program.
Prioritizes patient care needs, within established patient population and individually, to optimize long term health and health outcomes.
Collaborates with providers and clinicians to utilize best practice, evidence based guidelines in order to develop systems for achieving optimal patient and family/caregiver outcomes.
Demonstrates a range of interventions/skills including counseling, coaching, motivational interviewing, coordination, education, referral and advocacy to gain access to services and resources to empower patients, family and/or caregivers to involve them in mutual goal-setting, planning, behavior changes, and providing them the highest quality care.
Delegates appropriate tasks to members of the health care team and follows up when necessary to assure the delegated task is completed.
Demonstrates the ability to coach patients and improve their health behaviors. Apply a flexible and creative approach to problem solving.
Provides the following services to the established patient population: a.) Schedules face to face visits with the patient/caregiver/ family, in various settings including the patient's home, as well as telephonically and electronically, on a regular and as needed basis; b.) Develops a comprehensive care plan based on evidence based practice for chronic disease management; c.) Manages and coordinates all transitions in care by communicating the care plan with other healthcare providers; d.) Follow up telephonically and/or with a face to face visit, in various settings including patient home, with designated patients who are discharged from a hospitalization within three business days; then continue weekly contacts with these patients for a minimum of 4 weeks. e.) Provides evidence-based patient education and/or facilitates referrals to PHC or community resources for disease specific education and self-management. f.) Facilitates patient/family shared decision-making by assuring that adequate information is available; g.) Shares clinical expertise and decision-making for generating improved patient outcomes, experiences, and patient's sense of well-being; h.) Assists the patient and family in the examination and clarification of their values within the context of the clinical situation; i.) Demonstrates an understanding of the unique meaning of health, illness and disease for individual patients and family/caregiver; j.) Promotes healing by helping the patient and family cope with the fears and concerns that accompany life and healthcare changes; k.) Identifies the clinical situation by recognizing patterns and similarities in a problem to determine appropriate outcomes; l.) Demonstrates foresight in anticipating problems and intervenes proactively based on the plan of care; m.) Prioritizes quickly in unpredictable situations and adjusts strategies accordingly; n.) Negotiates conflicts by focusing on patient outcomes and promoting collaboration; o.) Identify patients' barriers to disease self-management and self-care and the additional services and education patients may need; p.) Identifies the dilemmas and offers constructive coping strategies; q.) Communicates clearly and convincingly to make the clinical case in order to obtain timely and appropriate responses based on the patient's needs; r.) Establishes trusting relationships with patients and families by active listening and mental and emotional presence with an understanding of the patient situation; s.) Empowers the patient through outreach and education and fosters the strong bond between the patient and the primary care provider
Must have BSN or currently enrolled in BSN completion program.
5 years of hospital-based nursing experience, adult primary care clinic or home care experience required.
Basic understanding of computer use, Microsoft Outlook, Microsoft Office programs, internet and health information technology (EHR).
Licensure: Registered Nurse (RN) WI or eNLC State
Case management, team lead or other nursing leadership experience is preferred
Preferred licensure: Guided Care Nursing, Nursing Case Management, Gerontological Nursing
