Communicates and documents accurate clinical and financial information in both verbal and written forms in order to assist Case Manager and or Social Worker in appropriate assessment, planning, implementation and evaluation of the patients continuing care needs. Interprets and communicates payer requirements to Case manager, Social Worker, patient, physician. Receives and documents benefit and contract level requirements in order to assure appropriate utilization of benefits and services. Supports the case management outcome model through clinical, clerical and support services in order to assist with the planning, assessment and implementation of appropriate patient transitions. Accurately arranges services for assigned patient population including Home Health, Durable Medical Equipment, ambulance, housing, skilled nursing facility and others in order to assure appropriate continuing care services are in place for patient transitions.
