Clinical Nurse Case Manager- Palliative Care Program

This position is responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach and assures quality, cost effective care for the identified patient population.
Minimum Qualifications: Associate Degree or diploma required, BSN preferred. Current valid Registered Nurse License in the State of Texas. Basic Life Support Certification required.
5 years experience as a Registered Nurse in the Clinical area assigned. 1-2 years experience with Case Management, Quality Management, Coding or Discharge Planning preferred.
2 years management/supervisory experience preferred.
Special Requirements: Communication Skills: Exceptional Average Verbal (Public Speaking), Writing/Composing Corespondence/Reports
Analytical, Mathematics, Medical Terms, P.C., Research, Stastical and Word Processing
Bilingual Skills Required: Spanish preferred
Advanced Education/Training: Case Management Certification desirable (CCM, CRRN, etc.) BSN preferred. Nursing or healthcare related degree desirable
Must have Basic level computer application experience/skills (Excel Word, etc.)
Work Schedule: Weekends, Overtime, Travel and On Call
