The Outreach Social Worker is assigned to the Veterans Affairs (VA) Medical Center and provides services to patients/significant others treated in the Homeless Veteran Program. The HealthCare for Homeless Veterans Program is a CARF accredited program. The Outreach Social Worker has extensive knowledge of Commission on Accreditation of Rehabilitation Facilities (CARF) standards and how to apply to the homeless Veterans program and services provided. Major responsibilities and duties include: Engages in psychosocial/diagnostic assessment with patient/family on each active case. Establishes therapeutic rapport and elicits relevant information by using interviewing skills and observation. Initiates outreach to the community, campsites, soup kitchens, local community shelters, to identify Homeless Chronically Mentally Ill (HCMI) Veterans. Provides a full range of social work services within commonly accepted standards of social work practice, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. Collaborates with local homeless coalitions, city officials, community shelter staff, community organizations, to maximize resources for Veterans. Provides all necessary direct and indirect care services consistent with the area of assignment. Will provide individual, family and group counseling as appropriate. Will act as liaison between VA and community resources, to include marketing and public relation duties as needed. Provide wellness/prevention education and facilitate Patient/family support groups as appropriate. As appropriate, supervise the practice of social work students, or a less experienced social worker. Provide psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Work Schedule: Monday-Friday 7:30AM-4:00PM or 8:00AM-4:30PM Financial Disclosure Report: Not required
