This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The VA Northern California Health Care system is currently recruiting a .5 part time (20 hours per week) Senior Social Worker- Hospital in Home for the Social Work service in the Mather location. The Senior Social Worker coordinates a variety of activities including home visits, telephonic assessment, outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to Medical Center staff and community based service providers, the collection and maintenance of statistical data, program planning evaluation and modification, and the supervision of students as assigned. Major duties include, but are not limited to: Develop an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, and family members/significant others. Contribute to the development of the treatment plan and setting achievable treatment goals with the veteran, family members/significant others and other clinical staff. Collaborate with other service providers in reassessing the veteran's needs for non-institutional, institutional services/programs and entitlements. Educate the veteran and families/significant others of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker. Make rapid assessments and develop crisis management plans. Provide education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans and families/significant others to the appropriate interdisciplinary team member for identified health education needs. Coordinate referrals of non-institutional services such as Medical Foster Home, ADHC/CADHC, home hospice and skilled and non-skilled homecare services in collaboration with the interdisciplinary treatment team members. Provide the veterans and their caregivers with ongoing supportive counseling. Conduct an intake health status assessment or an update assessment for OEF/OIF veterans; utilize clinical reminders to evaluate the need for health care, behavioral and mental health services; make appropriate referrals for health care, individual, group, marital and/or family treatment services.
