JOB SUMMARY: The Referral Center Clinical Coordinator (RCCC) facilitates the collaborative interdisciplinary process of case management encompassing assessment, planning, facilitation, care coordination, evaluation appropriate to the scope of licensure. The RCCC advocates for options and services to meet the patient's and family's comprehensive health needs through communication and available resources to promote quality cost-effective outcomes for the identified patient population.
The RCCC participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing Service FIRST behaviors in interactions with patients, families, and staff members.
MINIMUM QUALIFICATIONS:
Education/Licensure/Specialized Training:
a. Education: Graduation from an accredited school of Nursing with a Bachelor's degree in Nursing required. Master's Degree: Preferred
b. Licensure/Certification: Licensed to practice nursing in the State of Texas. Relevant clinical specialty certification required within two years of employment.CCM certification preferred
Work Experience: Five (5) years RN experience as appropriate to area assigned;One (1) year experience in Case Management, Quality Management, Coding or Discharge Planning preferred.
Management/Experience: One (1) year leadership experience preferred.
Equipment Operated: PC and typical medical equipment related to job duties.
