The role you’ll contribute:
Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. The Transitional Care Coordinator, in partnership with patients, caregivers, physicians, nursing and the multidisciplinary healthcare team, utilizes professional skills to reduce unnecessary readmissions. Coaches patients and families in disease self-management and establishes plans for safe and effective transfers in the movement of patients across the continuum of care. Promotes and evaluates the effective utilization of resources using current clinical knowledge, awareness of community services to achieve optimal clinical and resource outcomes.
PRIMARY CUSTOMERS, PARTNERS, AND TEAM: At AdventHealth Wesley Chapel, our primary customers are patients and families. Our partners are the physicians and our team members are all the employees of AdventHealth Wesley Chapel.
The value you’ll bring to the team:
Identifies patients with high risk conditions and circumstances for readmission and facilitates decision-making and communication to ensure resolution of care issues and provide comprehensive discharge plan with the goal of preventing readmissions (i.e.Heart failure, COPD, Pneumonia,Acute Myocardial Infarction patients, Joint replacement, and CVAs ; dressing changes; care of internal devices; what constitutes an emergency and when to notify the physician) with the patient and/or family using the ‘teach back” method.
Reinforce medication education given by Nursing and Pharmacist for population identified in #1 (coordinate with Nursing and Pharmacy).
Coordinate care of patients at risk for readmission from admission through 30 days post-discharge for defined patient population. Provide telephone reinforcement of the discharge plan and problem solving after discharge. Will contact patients that fall within a defined diagnosis. Reinforces education with the patient/family on plan of care and expected outcomes.Documents patient education in the electronic medical record (EMR).
Reviews and does an analysis of readmission prevention process.
Complete monthly reports on readmissions that includes the causes of readmission (such as related/unrelated) and the percentage of readmissions.
Participation in Readmissions meetings and webinars; assist in education of nursing staffing concerning readmissions.
Demonstrates knowledge of the principles of growth and development over the lifespan and possess the ability to assess data reflective of the patient’s status. Possess the ability to interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs and to provide the care needed.
Successfully manage a large patient load (26+ per month).
Qualifications
The expertise and experiences you’ll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
Associate of Arts in Nursing or Diploma in Nursing upon hire
3 years of experience in acute care upon hire
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current Florida RN license upon hire
