The RN Care and Care Coordinator demonstrates leadership through collaboration with the Geriatric and Extended Care team Team in developing patient-driven holistic care plans. This includes chart reviews, calls for pre-visit/post visit follow up, collaborative management of high-risk patients, and follow up with discharged patients. Provides patient and family health education focusing on self-management, prevention, and wellness, based on patient goals.
